(1 star) * - Familiar with. Can navigate through help screens and get something done.

(2 stars) ** - Have been around the program for a while but not used extensively.

(3 stars) *** - Comfortable with and can decent results fairly quickly.

(4 stars) **** - Have used for a long time - very comfortable and good workflow.

(5 stars) ***** - Guru, Master, Top Gun, etc.

Although not a complete list, I've tried to include the most common industry applications. I've worked around computers & software for a long time and have worked with literally hundreds of applications. I can generally learn new programs pretty quickly.
